About the UCH Ibadan SHIM Post UTME Form
Applications are invited by the management of the School of Health Information Management of the University College Hospital, Ibadan from suitably qualified candidates for admission into a Two-Year Programme which will lead to the award of the National Diploma (ND) in Health Information Management.

UCH Ibadan Entry Qualification for National Diploma (ND) Programme
The management of the institution expects candidates who wish to apply for admission into the National Diploma (ND) Programme to meet certain eligibility requirements. These are the requirements that make candidates eligible to apply for the Post UTME:
- Candidates who scored a minimum of 150 in the 2020 Unified Tertiary Matriculation Examination (UTME).
- Candidates who have obtained at least five(5) credits at not more than two sittings in SSCE/GCE/WASC/NECO. The results must be obtained from the same examination body and the required subjects include:
- English Language
- Mathematics
- Biology
- Physics or Chemistry
- Any other subject
Please Note: The following are to be taken into consideration:
- During the course of the training, pregnancy is not allowed.
- Immediately after admission is given, selected candidates will be required to undergo and pass a medical examination.
The course is non-residential and as announced by the school management will commence in October 2020.

Method of Application
National Diploma (ND) candidates who meet the eligibility requirements are required to pay a non-refundable fee of N2,000.00 (Two Thousand Naira), while National Diploma II (ND II) will be applying via Direct Entry and are required to pay the sum of N5,000.00 (Five Thousand Naira) for screening. The fees are to be paid into TSA Account (TSA/UCH/SHIM/ADMISSION FORM) in any commercial bank.
The bank teller that will be issued after payment is to be converted into a receipt by the applicant at the Cash Office of the Finance & Accounts Department, University College Hospital, Ibadan. Afterward, applicants are to forward the photocopy of the payment receipt and the photocopy of the bank teller to the Administrative Office of the School.
All applicants are to submit an active and functional email address. Applicants should know that they are expected to collect their access code from the school Administrative Office to enable them to apply online. To apply, candidates are to log on to www.portal.uch-ibadan.org.ng and submit their application after filling the details.
The screening exercise will be communicated later to candidates for the National Diploma (ND) entrance examination.
Through text messages and the School’s website, candidates who are successful will be invited for an oral interview.
Application Deadline
The deadline for application is stipulated for 31st August 2020.
Note: Wearing of Nose masks is compulsory for all candidates.
